Important!: Filling out this form does not book your evaluation but is a required step. In order to finish booking your evaluation please do so below. The $150 evaluation fee must be paid to receive an appointment confirmation.

Your evaluation will not be complete until your $150 payment is made. Once paid you will receive a appointment confirmation within 24 hours.
